2017-2018 Application Information and Instructions

For purposes of this application, the Northeastern region includes the following counties: Brown, Door, Florence, Forest, Kewaunee, Langlade, Lincoln, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Wood.

Community Team Application

Complete one application per team. Here is downloadable Word version for teams to work collaboratively prior to online submission.

Team Member Form (to be completed by each team member)

All team members should complete the Team Member Form. Each team member should describe what they can contribute to the team and coalition/partnership and what they would like to gain by participating in the Community Teams Program.

One team member should indicate that they are the primary contact for your team. Primary contact responsibilities include:

  • Serving as communication point person and liaison with HWLI
  • Scheduling and coordinating site visits 
  • Ensuring team members are participating in CTP activities
  • Serving as contact person for 20-month follow-up evaluation 

Healthy Wisconsin Leadership Institute

The Healthy Wisconsin Leadership Institute offers continuing education and training opportunities for those working to improve the health of the people of Wisconsin. Its overall goal is to build public and community health skills and leadership capacity throughout Wisconsin. The Health Wisconsin Leadership Institute is a professional development resources supported by the University of Wisconsin School of Medicine and Public Health's Wisconsin Partnership Program. 

Community Teams Program

The purpose of the Community Teams Program is to facilitate the development of the leadership and public health skills required to improve community health. The program is a ten-month educational experience for teams representing community coalitions or partnerships. Learning activities include 3 two-day workshops, webinars, consultation with program staff and experts, opportunities to visit and learn from other communities, and application of skills through a team action plan. 

The workshops provide guidance and instruction in the theory and practice of community health improvement and collaborative leadership. The sessions are designed for adult learners and include discussions, interactive exercises, and team activities.  

Between workshops, teams work on health priorities within their own communities. Program staff visits each team at least three times during the year to provide orientation, consultation and support. These visits take place prior to each workshop (Fall, early-winter and mid-spring). 

Regional Approach

The Community Teams Program will focus on one region each year to facilitate connections and disseminate success between communities. For purposes of this application, the Northeastern region includes the following counties: Brown, Door, Florence, Forest, Keewaunee, Langlade, Lincoln, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Wood.

Statewide teams are still welcome to apply, but all workshops will be held in one region each year. 

  • 2017-18: Northeast
  • 2018-19: Southeast

Building Skills

Through participation in this program, learners have an opportunity to:

  • Develop individual and collaborative leadership skills
  • Build strong partnerships and coalitions
  • Apply community health improvement models
  • Use practical evidence-based approaches to address health issues
  • Enhance action planning and evaluation skills
  • Practice storytelling for community health improvement
  • Improve ability to successfully compete for grant funding
  • Benefit from access to a network of colleagues and other experts

 Tools for Taking Action

Participants apply tools related to the community health improvement process. Teams learn how to effectively engage community in local health improvement efforts. This includes:

  • Identifying the root causes of the problem
  • ŸAnalyzing the scientific evidence supporting various intervention approaches
  • ŸEngaging coalition and community stakeholders in selecting a priority approach
  • ŸDeveloping an action and evaluation plan
  • Creating a sustainability plan

Here are some of the successes of past teams:

Northwoods LEAN (Linking Education, Activity, and Nutrition) further developed its coalition and received funding to implement organizational, policy, and environmental level changes to support residents who are motivated to increase physical activity and improve nutrition. Members of the team were selected to participate in the National Leadership Academy for the Public’s Health.

 The Beloit Lifecourse Initiative for Healthy Families (LIHF): Pathways to Healthier African American Families Team developed an action plan to increase cultural competency in their healthcare providers as part of a larger plan to reduce preterm births and decrease racial disparities in birth outcomes for African Americans in Beloit.

 re:Th!nk, Winnebago’s Healthy Partnership reorganized and restructured their coalition, built capacity of its Leadership Committee, and created multiple subcommittees. They went on to be recognized as the Outstanding Coalition of the Year at the WI State Prevention Conference and were also awarded a Community Transformation Grant.

The Healthy People Wood County Team implemented a comprehensive health and wellness program in a local elementary school and worked to sustain and expand community initiatives and resources aimed at improving physical activity and nutrition among Wood County residents.


Eligible communities:

  • Can be defined in terms of geographic boundaries (Northeast for 2017) or shared interests and must be within Wisconsin.
  • Will describe the process (e.g. surveys, focus groups, key informant interviews, data analysis, literature reviews or other assessment activities) for selecting the priority health issue to be addressed.
  • Will have initiated a coalition or partnership to address a community health issue that relates to one of the health priorities described in the state health plan.

A coalition is a group of individuals and/or organizations with a common interest working together toward a common goal.

Community Teams should be made up of 5 – 8 members who:

  • Are actively engaged in providing leadership for the community coalition.
  • Represent at least five different community sectors (preference is that members represent different organizations).  Partners include, but are not limited to, community and business leaders; employers; community health and primary care providers; health departments; hospitals, clinics, and health systems; media; law enforcement; policy makers; schools and educational organizations; faith-based organizations; volunteer or nonprofit community organizations; and individual residents.

Team Member and Team Responsibilities

All team members must agree to all of the responsibilities outlined below.

  • Commit time to complete the program (averages 15 hours per month over the 10-month period)
  • Actively participate in the orientation, webinars, site visits, 3 two-day workshops, and visits to other communities
  • Contribute to developing and initiating the team’s action plan
  • Identify own learning needs by completing a self-assessment
  • Identify team learning needs and expected benefits to the team
  • Cover travel and lodging expenses for workshops
  • Obtain employer permission to participate, if applicableŸ 

Tips for Putting Together an Effective Team

Policies (laws, rules, or norms) have significant impact on healthy behaviors and health promotion. With that in mind, consider including a policy-maker or those with access to policy-makers on your team. This could be an elected county board or school board member, school or hospital administrator, or an individual who sets policies within a private company or organization.

Invite members who represent your community and have a commitment to the health priority you have identified. It is important to include those who can contribute unique perspectives and resources to your priority health issue due to their professional affiliations, cultural backgrounds, work experience, personal experience or other characteristics. 

Team Selection

A workgroup of the Healthy Wisconsin Leadership Institute’s Advisory Committee will conduct a review of all applications and then select teams for 2017-2018.

Reviewers will score applications using a rubric based on the following criteria:

  • Demonstration of health priority and community need.
  • The extent to which the team’s composition is multi-sectoral.
  • The extent to which the objectives and learning needs of the coalition/partnership match the Community Teams Program curriculum.

All applicants receive reviewer feedback.

Continuing Education Credits

The program offers University continuing education credits.

Program Costs

The Leadership Institute covers the cost of instructional materials and programming, facility expenses for workshops, and fees for guest faculty and experts who provide technical assistanceTeam members are responsible for all travel, lodging, and costs associated with their participation.
