Community Teams Application for 2012-2013

Application Deadline: June 15, 2012

Teams are encouraged to apply for the program each spring. The application information and instructions provide information on the program, team eligibility criteria and how to apply. Once you have reviewed the application materials, please apply online using the secure form link below. Please direct any questions to program staff.

How to Apply

Form a team that meets the eligibility criteria
Complete the Online Team Application Form
Each team member, including the primary contact must read and sign the Signature Form and have approval and signature from their Employer

We strongly prefer that you apply online, however, if you prefer to apply on paper, you may download the forms below and mail or email your application.
