Community Teams Applications for 2013-2014

Teams are encouraged to apply for the program each spring. The application information and instructions provide information on the program, team eligibility criteria and how to apply. Once you have reviewed the application materials, please apply by downloading the forms below. Please contact program staff with any questions.

How to Apply

• Carefully review Program Information and Application Instructions (PDF).
• Form a team that meets the eligibility criteria.
• Select a primary contact person.
• Complete the Team Application Form (MS Word). Obtain Signature Forms (PDF) from each team member. It is recommended that the primary contact person collect all pertinent information from team members in order to complete the application.
